Primary Care Behavioral Health (PCBH) Model is an evidence-based model of integrated care that places behavioral health consultants (BHCs) embedded directly within a primary care clinical team. This is a collaborative, team-based approach to care where the BHC works alongside the PCP to manage a wide range of behavioral health concerns and biopsychosocial issues in a single, accessible location.
