Collaborative Care Model: Base Training is designed for Patient-Centered Medical Home (PCMH) practices that have completed the CoCM practice assessment and are determined to be ready to implement the model. The training is intended to help the primary care physicians, psychiatric consultants, behavioral health care managers, and other primary care practice team members gain the foundational knowledge and skills for implementing CoCM. After completing this activity, participants will be able to translate key processes within their practice setting and integrate CoCM into patient visits.

Collaborative Care Model (CoCM): Adolescent Training is built upon the content provided in the foundational training, CoCM: Base. This training is intended to help the primary care physicians, psychiatric consultant physicians, behavioral health care managers, and other primary care practice team members gain the foundational knowledge and skills for implementing CoCM with the adolescent population. After completing this activity, participants will be able to apply CoCM with the adolescent population, focusing on patient caseload, suicide assessment, and treatment of depression and anxiety.

Perinatal in Collaborative Care training is designed for practices that have completed the Collaborative Care Model (CoCM) Base Training and have completed the Perinatal Practice Assessment with a determination that the practice is ready for implementation. The training is intended to help the medical provider, psychiatric consultants, behavioral health care manager, and other practice team members gain the foundational knowledge and skills for implementing CoCM with the perinatal population.  After completing this activity, participants will be able to translate key processes within their practice setting and integrate CoCM into perinatal patient visits.

Collaborative Care Model: SUD training is designed for practices that have completed the Collaborative Care Model (CoCM) Base Training and has a provider and/or psychiatrist who has completed MAT training and/or is comfortable advising these conditions. The training is intended to support the management of AUD/SUD through having an increased understanding of the prevalence and why it is important to include AUD/SUD management in primary care, reviewing of and use of the screening tools used to identify individuals with at risk or with dependence and having a care team member(s) prepared to have a patient-centered objective and meaningful conversation when SUD/AUD risk are identified.
