Implementing Collaborative Care (“Base Training”) is designed for Patient-Centered Medical Home-designated practices that have completed the Collaborative Care Model (CoCM) practice assessment with a determination that the practice is ready for implementation. This training is intended to help the primary care physicians, psychiatric consultants, behavioral health care managers, and other primary care practice team members gain the foundational knowledge and skills for implementing CoCM. After completing this activity, participants will be able to translate key processes within their practice setting and integrate CoCM into patient visits.
The Implementing Collaborative Care with Adolescent and Pediatric Patients Training is built upon the content provided in the foundational training, Implementing Collaborative Care (“Base Training”). This training is intended to help the primary care physicians, psychiatric consultants, behavioral health care managers, and other primary care practice team members gain the foundational knowledge and skills for implementing Collaborative Care with pediatric and adolescent populations. After completing this activity, participants will be able to apply Collaborative Care with pediatric and adolescent populations, focusing on patient caseload, suicide assessment, and treatment of depression and anxiety and other common issues.
The Implementing Collaborative Care with Perinatal Patients training is designed for practices that have completed the Implementing Collaborative Care (“Base Training”) and have completed the OB/GYN Practice Assessment with a determination that the practice is ready for implementation. The training is intended to help the medical provider, psychiatric consultant, behavioral health care manager, and other practice team members gain the foundational knowledge and skills for implementing CoCM with the perinatal population. After completing this activity, participants will be able to incorporate workflows and other operational techniques unique to Collaborative Care within their practice to address the behavioral health needs of the perinatal population with fidelity to the Collaborative Care Model.
The Treating Substance Use in Collaborative Care Settings training is designed for practices that have completed the Implementing Collaborative Care (“Base Training”). The training is intended to help the medical provider, psychiatric consultant, behavioral health care manager, and other practice team members gain the foundational knowledge and skills to treat substance use disorders using the Collaborative Care Model. After completing this activity, participants will be able to incorporate workflows and other operational techniques unique to Collaborative Care within their practice to address the behavioral health needs of patients with substance use disorders with fidelity to the Collaborative Care Model.
