MICMT is offering one-on-one consultations to help strengthen team-based care and patient engagement throughout the state of Michigan by providing support to Physician Organizations and the care team members in their affiliated practices.

Evidence-based practice includes the integration of the best available evidence, clinical expertise and the perspective and preferences of the patient. To help treat certain clinical conditions evidenced based information can be found in an assortment of ways.

The purpose of the job board is to highlight outpatient care management or team-based care roles available in Michigan. This may include but not limited to nurses, social workers, dietitians, pharmacists, community health workers, and medical assistants.
