
Implementing Primary Care Behavioral Health Model (“Base Training”) is designed for Patient-Centered Medical Home-designated practices and Provider-Delivered Care Management practices that have completed the Primary Care Behavioral Health (PCBH) practice assessment with a determination that the practice is ready for implementation. This training is intended to help the primary care physicians, behavioral health consultants, and other primary care practice team members gain the foundational knowledge and skills for implementing PCBH.
After completing this activity, participants will be able to incorporate workflows and other operational techniques unique to Primary Care Behavioral Health within their practice to address the behavioral health and psychosocial needs of patients with fidelity to the Primary Care Behavioral Health Model. They will be able to implement the Primary Care Behavioral Health Model in their practices to address the behavioral health and psychosocial needs of the patient population.
