The Specialist Team-Based Care course is designed to provide care team members with an understanding of the Blue Cross Blue Shield of Michigan Specialist Team-Based Care Program and how teams work to support success in this program. Learn about processes and communication between Primary Care and Specialist offices to improve care coordination and patient outcomes.

Learning Outcome: Learners will be able to describe two processes that are recommended to support their success in the BCBSM Specialist Team-Based Care Program.

The course will take 60 minutes to complete. Successful completion of the Specialist Team-Based Care course requires:

Certificate of Completion (Available on MICMT's Website ONLY!)

1. Viewing the entire course (below).
2. Completing the post-test and evaluation and achieving a passing score of 80% or more. 
3. Once the evaluation and post-test is completed, the certificate of completion will post to your user dashboard.

  • You may retake the post-test if needed. 
  • As part of the evaluation, you will be required to attest to having viewed the course in its entirety.

Continuing Education and Continuing Medical Education Credit (Available on Beaumont/Corewell Health's CE/CME Portal) 

1. Reviewing the CE/CME handout prior to viewing the entire presentation. (REQUIRED). 
2. Completing the post-test and evaluation and achieving a passing score of 80% or more. 
3. Post-test, evaluation, and earned CE/CME certificate activity will take place on the Beaumont CE/CME portal. CE/CME certificates for this training will remain on Beaumont's CE/CME portal. 

  • You may retake the post-test if needed. 
  • As part of the evaluation, you will be required to attest to having viewed the course in its entirety.

This presentation is approved for AMA PRA Category 1 Credit™, ANCC Nursing contact hours, ACPE pharmacy contact hours, and Commission on Dietetic Registration (CDR) CPEUs through Beaumont Health.  

Upon successful completion of the Specialist Team-Based Care course, you may request a certificate of completion or download the CE/CME handout to review instructions on obtaining CE/CME credit.
