PCBH: Expanding Integrated Behavioral Health
Primary Care Behavioral Health (PCBH) is an integrated care model in which a Behavioral Health Consultant (BHC) is fully embedded within the primary care team to provide brief, population-based behavioral health services. PCBH is designed to support the entire primary care population with behavioral health care being delivered through real-time collaboration between the BHC and primary care providers within existing clinic workflows.
PCBH emphasizes rapid access, brief interventions, high patient volume, and team-based care to address behavioral health, emotional, and health behavior needs as part of routine medical care. The goal is to improve overall health outcomes by making behavioral health care accessible, efficient, and seamlessly integrated into primary care settings.
