Effective Patient Engagement Strategies and Tools for Care Team Members Series
This series will occur between August 10th through November 16th. Provides care-team members the opportunity to enhance their motivational interviewing knowledge. Each webinar will have practical application to patient scenarios such as care management enrollment, substance use disorder, advance care planning and weight management.
The goal for this education is to provide primary and specialist care teams a better understanding about the significant role of medications and diet in individuals with chronic kidney disease. A Certificate of completion is available after completing each session.
