Join us for our new four-part series on the 3rd Thursday of every month between July and October! We'll take a deep dive into understanding the multiple facets of transitions of care, the complexities of care that patients can be impacted by, and how best to support patients to ensure effective services.
This series will occur between August 10th through November 16th. Provides care-team members the opportunity to enhance their motivational interviewing knowledge. Each webinar will have practical application to patient scenarios such as care management enrollment, substance use disorder, advance care planning and weight management.
The goal for this education is to provide primary and specialist care teams a better understanding about the significant role of medications and diet in individuals with chronic kidney disease. A Certificate of completion is available after completing each session.
This training will create a foundation for introducing palliative care into the primary care setting earlier in the course of a serious illness.
