The 2024 Scorecard highlights elements that MICMT / BCBSM consider fundamental for team-based care and care management program success. This distribution will occur in the January 2026 PGIP check to allow time for outcomes evaluation. There are three identified measures:
- Outcomes
- Care Management Operations
- Engagement
The Scorecard is used to reward those POs participating in BCBSM's Provider Delivered Care Management. POs who have not demonstrated engagement and active participation will not be eligible to be rewarded.
