Primary care is often the first stop for people experiencing symptoms of behavioral health conditions, including anxiety. In this presentation, participants will learn the difference between stress and anxiety, how to identify somatic symptoms that may have roots in anxiety, and how to thoughtfully speak about anxiety and its impact. In addition, participants will receive information about effective, evidence-based screenings and interventions to use when working with patients with anxiety. We will address common myths about anxiety and leave time for questions.

A panel of PO representatives will be discussing how they are involved with post-acute care collaboratives, which aim to bring various partners (hospitals, skilled nursing facilities, home health agencies, etc.) together with a shared goal of improving the quality of care provided as patients move from the acute to post-acute setting. They will discuss how the collaboratives function and how ambulatory care managers are being used and connected in the process. Panelists will discuss the development of their collaboratives, as well as their on-going management and relationships between the PO and their partners.

The MI-POST is an optional form that documents the patient’s decisions and puts them into a physician’s order set that can be followed at any Michigan health care facility, as well as by first responders, and a key tool in the advance care planning process. This webinar will help participants: 
- Describe the POLST paradigm 
- Describe the MI-POST 
- Identify ways of communicating wishes regarding care 
- Identify appropriate uses of MI-POST 
- Troubleshooting conflict around MI-POST 

Additionally, the Michigan Department of Health and Human Services will provide updates on changes made to the MI-POST form in 2023.
