A panel of PO representatives will be discussing how they are involved with post-acute care collaboratives, which aim to bring various partners (hospitals, skilled nursing facilities, home health agencies, etc.) together with a shared goal of improving the quality of care provided as patients move from the acute to post-acute setting. They will discuss how the collaboratives function and how ambulatory care managers are being used and connected in the process. Panelists will discuss the development of their collaboratives, as well as their on-going management and relationships between the PO and their partners.

Functional and cognitive status, including aging and other life changes, often lead to more interdisciplinary team involvement and increased utilization of community resources. Tools, strategies, and types of available resources will be described.

From the moment we are born, we are aging. Understanding usual changes of aging are important to know in meeting the unique needs, values, and preferences of older adults. Participants will learn about aging including health promotion strategies that lead to improved quality of life and what really matters most.
