Collaborative Care (CoCM) is the most evidenced integrated behavioral health model. This Sustainability Roundtable will feature speakers from several POs implementing CoCM as part of the Blue Cross Blue Shield of Michigan (BCBSM) Physician Group Incentive Program (PGIP). Topics will address achieving and maintaining sustainability, including encouraging appropriate referrals, program performance, and billing. The intended audience for this event is PO leadership and PO CoCM leaders.
