A panel of PO representatives will be discussing the operations of their remote patient monitoring programs as well as how ambulatory care managers are being used and connected in the process. The remote patient monitoring programs seek to target and support patients with uncontrolled hypertension, type 2 diabetes, high risk discharges as well as patients with COPD, CHF, or on a cardiac pathway.
