OSP Care Management/Clinical & Community Resources Spotlight Meeting

The purpose of this meeting is to align Oakland Southfield Physicians care management teams on program updates, performance expectations, available tools and resources, and upcoming educational opportunities, while strengthening connections to community partners that support patient care. By the end of the meeting, participants will be able to: 

-Understand Care Management Program Updates Review annual changes to the care management program, including billing updates, engagement expectations, and outcomes targets, and understand how these changes impact care management workflows and performance.

-Leverage OSP Tools and Resources Identify tools for strategic patient targeting, clarify billing eligibility guidance, and access clinical, chronic condition, and community resource supports to enhance patient engagement and care coordination.

-Stay Informed on Clinical and Program Advancements Gain awareness of new public-facing diabetes resources and recent clinical advancements in Type 2 diabetes care that support improved patient outcomes.

-Plan for Training and Professional Development Review upcoming care management training opportunities, reinforce the annual continuing education requirement, and encourage proactive participation in learning opportunities.

-Prepare for Upcoming Initiatives and Events Learn about upcoming MICMT educational webinars and OSP care management–related events to support ongoing engagement and program readiness.

-Strengthen Community Partnerships Increase awareness of locally available community resources, understand how they support patients with identified needs, and explore opportunities to integrate these services into individualized care plans.
