This training will take place over two non-consecutive days.
Learners must successfully complete the full duration of this training on both days for credit.
This training will focus on medication reconciliation best practices across multiple care team settings, including primary care, specialty, care management focused visits, and transitions of care. A description of various team member roles will also be discussed in order to optimize the care they provide for patients in these settings.
