Optimizing Medication Reconciliation: Role of Care Team Members (July 16th and 23rd) (Archived) 2401

This training will take place over two non-consecutive days. 

This training will focus on medication reconciliation best practices across multiple care team settings, including primary care, specialty, care management focused visits, and transitions of care. A description of various team member roles will also be discussed in order to optimize the care they provide for patients in these settings. 

  • July 16th: Day 1 (11:00 am - 1:00 pm)
  • July 23rd: Day 2 (11:00 am - 1:00 pm)
