During this meeting, Care Managers will gain a better understanding of the upcoming changes associated with PCMH 2.0, with a focus on areas that directly impact their role, including transitions of care (TOC), community resource coordination, and required screenings. Care Managers will also have the opportunity to learn from other NPO practices by discussing what has been effective—and what has not—regarding diabetic eye exam workflows. In addition, we will review key concepts from Youth Mental Health First Aid and discuss how those principles can be applied when working with adult populations. The meeting will also include time for open discussion, allowing Care Managers to raise additional topics. (An email was sent in advance, so additional discussion points may emerge.) Finally, NPO will gather feedback on how recent billing changes are impacting workflows and operations.
