In the October Meeting, a PDCM billing report was discussed that showed which percentage of codes the practice/practices are billing the most. Areas of opportunity were shared; following up, this meeting will have best practice shared from Care Managers who have a higher probability of billing for areas of opportunity.
Care Managers can identify ways to help increase their billing and will also learn ways to start PDCM group visits if it is something of interest for them. Reminders will be given on Care Coordination codes and being able to utilize portal time for BCBSM.
Care Manager’s will receive Post Discharge phone scripts for Adults and Pediatrics to help with meeting the big picture- ED and Inpatient follow-up is essential to help ensure: appropriate ED use, making sure patients understand their discharge instructions, red flag symptoms to look out for when considering diagnosis and or medication changes, ensuring patient needs are met, and can be great opportunities to discuss Advanced Care Planning concepts; all of these things can help reduce inappropriate ED use, readmissions and appropriate care aligning with patient wishes when serious illness or end of life is a factor.
The Phone Scripts include billing prompts, red flags for frequent admission diagnosis, medication counseling points, and lots of open-ended questions to make sure everyone is on the same page and get to the root of needs.
