NPO Care Manager Meeting

During this meeting Care Managers will learn about PCMH and how PCMH and PDCM tie together. It will make them aware of other facets in the office and similar to a presention PCMH practice managers recieved 2 weeks ago but there will be more focus on Care Management in the PCMH, 2025 PCMH changes, Best Practices (encouraging to pick one thing and work on it). NPO is also interested in holding monthly MI sessions if there is interest. Finally a CM recently reported using the "ask the pharmacist" MICMT resource and found it very helpful. She will share her experience and Rachael, NPO will remind/show the CMs how to access this resource
