During the 2024 Best Practice Kickoff, Practices and processes will be reviewed that were gone over during the 2023 year. Care Managers will be encouraged to pick one thing or area to focus on for 2024, at least starting out in 2024. This same presentation will be shared at an NPO PCMH User Group; each group will have a different twist. The Care Manager group will get a basic high-level overview of some PCMH concepts, be encouraged to talk to their practice managers, and more focus will be on the Care Manager specific topics such as billing opportunities, post discharge phone scripts, TOC/TCM, Med Rec etc. During the pharmacist corner Care Managers will receive demo on proper use inhalers, spacers etc. Not utilizing inhalers and spacers correctly can be big reasons for hospital readmission; medication counseling points for inhalers were shared during the December Care Manager meeting. This is building off of that presentation. Care Managers will also learn about the stages of CKD and medications that may help slow the progression.
