NPO Care Management Meeting

During the meeting, care managers will:

• Learn how to better assess patients’ blood pressures in the virtual setting, taking into to consideration things like body posture, current medications, and be reminded of things that may affect blood pressure such as recent smoking or caffeine
• Discuss direct admits from the PCP office to SNF; directions and steps for how to directly admit patients to SNF and which patients are appropriate.
• Review PCDM billing, identify most frequently billed codes and discuss areas of opportunity for billing.
