During the 211 presentation a demo will be given on how to use 211 services and how these services be utilized by practices including Care Managers to further assist with patient SDOH needs. Midwest Health Monitoring (RPM) and Northern Homecare are newer resources available to practices in the traverse city region. Information on these resources will be shared along how RPM can assist further in managing patients with Chronic Conditions. Northern Homecare can be a great resource for patients and certain SDOH needs. Last during the palliative care resource presentation, tool/resources will be shared from the MiCCSI Palliative Care training with focus on Serious illness conversation. A How to use these resources will be shared along with conversation and content such as “I wish statement” and other skills similar to Motivational Interviewing to assist with more meaningful serious illness conversation. Video examples will also be shared regarding an optimal and less optimal serious illness conversation example.
