Next Steps: Respecting Choices: ACP Facilitator Training

The NEXT STEPS program is the second in the series of the Respecting Choices curriculum that promotes communication skills useful in assisting patients in planning for the future when chronic illness has become more advanced and complications, frequent hospitalizations and decline in function are occurring. These communication skills are important in setting goals and managing these chronically ill patients.
