Navigating Complex Cases in CoCM

Navigating Complex Cases in CoCM explores how collaborative care teams can effectively manage challenging clinical situations that test the boundaries of the model. Through discussion and case examples, participants will examine what makes a case “complex” or “difficult”—including issues of diagnostic complexity or uncertainty, challenges in patient engagement, social or systemic barriers, and poor response to treatment. The session will highlight strategies for enhancing team communication, adapting evidence-based interventions, and leveraging the unique roles of the Primary Care Provider, Behavioral Health Care Manager, and Psychiatric Consultant to improve outcomes for complex patients.
