Olympia Medical, LLC has been approved by the Michigan Institute for Care Management and Transformation (MICMT) to provide the MICMT Introduction to Team-Based Care course training. This course is required to bill care management/care coordination codes. Introduction to Team Based Care helps the learner to provide the right care, to the right patient, in the right setting and at the right cost, through a dedicated multidisciplinary care team and in collaboration with the patient, effectively managed in a primary care setting. Open to all members of the practice to gain foundational knowledge in Team Based Care. Course Content Includes: • Care Model and Quality • Team Based Roles • Care Management Process • Sustainability • Coding
