Practice Transformation Institute (PTI) has been approved by the Michigan Institute for Care Management and Transformation (MICMT) to provide the MICMT Introduction to Team-Based Care course training.
Introduction to Team-Based Care helps the learner provide the right care, to the right patient, in the right setting and at the right cost, through a dedicated multidisciplinary care team and in collaboration with the patient, effectively managed in a primary care setting. Open to all members of the practice to gain foundational knowledge in Team-Based Care.

- Team-Based Roles
- Care Management Process
- Sustainability
- Coding
This training takes place over 2 days. Attendees must complete both days in order to receive credit.
Day 1: Tuesday, August 3 from 8am-12:30pm
Day 2: Wednesday, August 4 from 9am-11:30am
Training cost per person: $300
