Introduction to Team-Based Care (Archived) 1090

Practice Transformation Institute (PTI) has been approved by the Michigan Institute for Care Management and Transformation (MICMT) to provide the MICMT Introduction to Team-Based Care course training. This course is required to bill care management/care coordination codes. 

Note: This course takes place over two days. Full attendance is required on both days in order to receive credit.

  • Day 1: May 5, 2021 – 8 a.m. to 12:30 p.m.

Introduction to Team-Based Care helps the learner to provide the right care, to the right patient, in the right setting and at the right cost, through a dedicated multidisciplinary care team and in collaboration with the patient, effectively managed in a primary care setting.

Open to all practice members to gain foundational knowledge in Team-Based Care. Course content includes:

  • Care Model and Quality
  • Team Based Roles
  • Care Management Process
  • Sustainability
  • Coding
