Stellar Health Olympia Midwest has been approved by the Michigan Institute for Care Management and Transformation (MICMT) to provide the Introduction to Team-Based Care course training.
The Introduction to Team-Based Care course helps the learner to provide the right care, to the right patient, in the right setting and at the right cost, through a dedicated multidisciplinary care team and in collaboration with the patient. This course is open to all dedicated care team members of primary care and specialty practices, to gain foundational knowledge in team-based care. Introduction to Team-Based Care will include:
- Why, What, Who and How: Team-Based Care
- Care Management Process
- Outcomes and Quadruple Aim
- Selecting Appropriate Codes to promote Sustainability
This course will take place over three days; attendance is required on all three days for credit:
- Tuesday, March 3: 9:00 – 11:30 a.m.
- Wednesday, March 4: 9:00 – 11:00 a.m.
- Thursday March 5: 9:00 – 11:00 a.m.
Cost of Training:
- Stellar Health Olympia Midwest member practices: Free
- Non-Stellar Health Olympia Midwest member participants:
- Single package: $300 per person
- Bundle package (Introduction to Team-Based Care and Patient Engagement courses): $270 per person per course (10% discount)
