Introduction to Team-Based Care

Michigan Center for Clinical Systems Improvement (Mi-CCSI) has been approved by the Michigan Institute for Care Management and Transformation (MICMT) to provide the MICMT Introduction to Team-Based Care course training. This course is required to bill care management/care coordination codes.
   
During this live, all-day training course the learner will gain knowledge and skills to work collaboratively in a multidisciplinary setting to assure that care is coordinated, safe, and patient-centered. Open to all members of the practice to gain foundational knowledge in Team-Based Care. Introduction to Team-Based Care will include:

   • Why, What, Who and How: Team-Based Care
   • Care Management Process
   • Outcomes and Triple AIM
   • Billing Applications
 

At the conclusion of this training course, the participant will be able to:
  

1. Effectively empower patients and their families.
2. Engage patients in self-management and health behavior change.
3. Positively affect patient self-care practices and health outcomes.
4. Provide comprehensive assessment including a holistic evaluation social determinants of health and care planning using shared decision making.
5. Implement evidence based interventions and advocate for the right care, at the right time and in the right place.

4. Complete the prework.
