Introduction to Team-Based Care

Northern Michigan Care Partners has been approved by the Michigan Institute for Care Management and Transformation (MICMT) to provide the MICMT Introduction to Team-Based Care course training. This course is required to bill care management/care coordination codes.

This will be a general orientation to the team-based care model, care management roles, billing basics and outcomes. 

Participants are required to have access to a camera and a microphone during this training.
