Practice Transformation Institute (PTI) has been approved by the Michigan Institute for Care Management and Transformation (MICMT) to provide the MICMT Introduction to Team-Based Care course training. This course is required to bill care management/care coordination codes.
Introduction to Team-Based Care helps the learner to provide the right care, to the right patient, in the right setting and at the right cost, through a dedicated multidisciplinary care team and in collaboration with the patient, effectively managed in a primary care setting. This training is open to all members of PTI to gain foundational knowledge in Team-Based Care.
Successful course completion includes:
- Attendance throughout the entire course
- Use of a webcam during training is strongly encouraged
- Completion of the course post-test: need to have a score of 80% or greater on the post-test
- Completion of the course evaluation
