Introduction to Team-Based Care (Archived) 965

IHA has been approved by the Michigan Institute for Care Management and Transformation (MICMT) to provide the MICMT Introduction to Team-Based Care course training.

The Introduction to Team-Based Care course helps the learner to provide the right care, to the right patient, in the right setting and at the right cost, through a dedicated multidisciplinary care team and in collaboration with the patient, effectively managed in a primary care setting. Open to all members of the practice to gain foundational knowledge in Team-Based Care.

Introduction to Team-Based Care will include:

• Care Team Model and Team Based Roles
• Care Management Process
• Outcomes
• Selecting Appropriate Codes to Promote Sustainability
