Introduction to Team-Based Care

The Michigan Primary Care Association (MPCA) has been approved by the Michigan Institute for Care Management and Transformation (MICMT) to provide the MICMT Introduction to Team-Based Care course training. This course is required for practices billing care management/care coordination codes.  

The “Introduction to Team-Based Care” course is designed to provide learners with a better understanding of how to work in a multidisciplinary care team and collaborate with patients effectively. This course is open to all care team members (nurses, MAs, CHWs, LMSW, dietitians, pharmacists) who wish to gain foundational knowledge in Team-Based Care. The course will cover a range of topics related to Team-Based Care and equip learners with the necessary skills to work in a collaborative care team environment.

  • Care Management Process 
  • Outcomes and Triple/Quadruple AIM 
  • Provider Delivered Care Management (PDCM) 
  • Billing Applications
