Introduction to Team-Based Care

CIPA has been approved by the Michigan Institute for Care Management and Transformation (MICMT) to provide the MICMT Introduction to Team-Based Care course training. This course is required for practices billing care management/care coordination codes.  

This course will take place over three non-consecutive days; attendance is required on 3 days for credit, as well as audio AND video capability:

Monday, October 21st: 1:30 pm – 3:30 pm
Monday, October 28th: 1:30 pm – 3:30 pm
Monday, November 4th: 1:30 pm – 3:30 pm

The Introduction to Team-Based Care course helps the learner better understand how to work in a multidisciplinary care team and in collaboration with the patient. Open to all members of the practice to gain foundational knowledge in Team-Based Care. Introduction to Team-Based Care will include:

  • Why, What, Who and How: Team-Based Care
  • Care Management Process
  • Outcomes and Triple AIM
  • Billing Applications
