Introduction to Team-Based Care (Archived) 877

The Introduction to Team Based Care course is designed to prepare the care team for the delivery of complex care management in the ambulatory care setting. Through this course, the care team will learn the framework for the complex care management role, foundational elements of integration into the ambulatory care setting, and incorporation of complex care management skills. Course content includes:

• Care Management Delivery in the Primary Care Setting
• Team Based Care
• The Five Step Care Management Process
• Social Determinants of Health
• Comprehensive Assessment and Care Plan
• Billing and Financial Stability

Virtual Live Training - over the course of 3 days (approx. 2 hrs per day)

• November 9th (8:30 am - 11:00 am)
• November 10th (8:30 am - 10:30 am)
• November 11th (8:30 am - 10:30 am)

Cost of training is $250 per person for non-Olympia Medical, LLC member practices
