Cure-Michigan has been approved by the Michigan Institute for Care Management and Transformation (MICMT) to provide the MICMT Introduction to Team-Based Care course training.
This course will take place over two non-consecutive days; attendance is required on both days for credit, as well as audio AND video capability:
- Friday, February 24: 8:30 a.m. – 12:30 p.m.
- Wednesday, March 1: 8:30 a.m. – 12:30 p.m.
This course is intended for nurses, social workers, advanced practice providers, medical assistants and community health workers who are new to care management or care coordination, or seeking to expand their knowledge and skills as part of a Patient Centered Medical Home, Accountable Care Organization, or other transitional care role. Attendees will enhance their knowledge and skills for collaborative team work in a multidisciplinary setting improving patient-centered care.
Dr. Lynn Klima DNP, CNE, APRN- ANP-BC is the trainer for this course, and is well versed in the concepts and has extensive experience to offer insight and application of the skills, tools, and information covered.
At the conclusion of this course, the participant will be able to:
- Effectively empower patients and their families.
- Engage patients in self-management and health behavior change.
- Positively affect patient self-care practices and health outcomes.
- Provide comprehensive assessment including a holistic evaluation social determinants of health and care planning using shared decision making.
- Implement evidence based interventions and advocate for the right care, at the right time and in the right place.
