Introduction to Team-Based Care (Archived) 1830

Practice Transformation Institute (PTI) has been approved by the Michigan Institute for Care Management and Transformation (MICMT) to provide the MICMT Introduction to Team-Based Care course training. This course is required for all care team members new to their role in order to bill PDCM codes.

Note: This training takes place across 2 days. You must attend both days to receive credit.

• Day 1: Wednesday, July 19 – 8 a.m. to 12:30 a.m.
• Day 2: Thursday, July 20 – 9 a.m. to 11:30 a.m.

This course will include:

• Why, What, Who and How: Team-Based Care
• Care Management Process
• Outcomes and Triple AIM
• Billing Applications

Training Cost: $300 per person
