This training will provide care team members with a foundation by which to begin introducing Palliative Care into the primary care practice. Course Content Includes:

• An introduction to Palliative Care
• Focus on the Eight Domains of Palliative Care
• The Multidisciplinary team
• Communicating Palliative Care to patients and families

Successful course completion includes:

• Attendance for the entire course
• Use of webcam during training is strongly encouraged
• Completion of the course post-test: need to have a score of 80% or greater on the post-test
