This webinar will discuss how Oakland Southfield Physicians (OSP) activated health information exchange to promote and scale patient-centered care coordination. Participants will learn how OSP used innovative workflow, supported by the practice care team, to increase the adoption of Admission, Discharge, and Transfer (ADT) and Consolidated–Clinical Document Architecture (CCDAs) within our care community.
