Implementing Primary Care Behavioral Health (“Base Training”) is designed for Patient-Centered Medical Home-designated practices and Provider-Delivered Care Management practices that have completed the Primary Care Behavioral Health (PCBH) practice assessment with a determination that the practice is ready for implementation. This training is intended to help the primary care physicians, behavioral health consultants, and other primary care practice team members gain the foundational knowledge and skills for implementing PCBH.
Objectives:
Upon completion of this activity, participants will be able to:
- Incorporate workflows and operational techniques unique to the Primary Care Behavioral Health Model to address the behavioral health and psychosocial needs of their patients with fidelity to the model.
- Implement the Primary Care Behavioral Health Model within their practices to effectively meet the behavioral health and psychosocial needs of their patient population.
