Implementing Collaborative Care (“Base Training”) is designed for Patient-Centered Medical Home-designated practices that have completed the Collaborative Care Model (CoCM) practice assessment with a determination that the practice is ready for implementation. This training is intended to help the primary care physicians, psychiatric consultants, behavioral health care managers, and other primary care practice team members gain the foundational knowledge and skills for implementing CoCM.
After completing this activity participants will be able to incorporate workflows and other operational techniques unique to Collaborative Care within their practice to address the behavioral health needs of patients with fidelity to the Collaborative Care Model. They will be able to implement the Collaborative Care Model in their practices to address the behavioral health needs of the patient population.
