Implementing Collaborative Care (2027)

Implementing Collaborative Care (“Base Training”) is designed for Patient-Centered Medical Home-designated practices that have completed the Collaborative Care Model (CoCM) practice assessment with a determination that the practice is ready for implementation. This training is intended to help the primary care physicians, psychiatric consultants, behavioral health care managers, and other primary care practice team members gain the foundational knowledge and skills for implementing CoCM. 

Objectives

  • Incorporate workflows and other operational techniques unique to Collaborative Care within their practice to address the behavioral health needs of patients with fidelity to the Collaborative Care Model.

  • Implement the Collaborative Care Model in their practices to address the behavioral health needs of the patient population.

August 3rd Day 1: 8:00 AM - 4:00 PM

  • Providers and Psychiatric Consultants will only be required to attend Day 1 from: 8:00 AM - 11:45 AM

August 5th Day 2: 8:00 AM - 4:00 PM

Target Audience: Physicians, social workers, nurses, administrators, and primary care practice team members participating in CoCM.
