Holland PHO Team Based Care Management, Codes, and Billing Workshop

The training will be an interactive in-person workshop style presentation sharing best practice to optimize team-based care management activities, bill for services, build a sustainable practice, and promote quality outcomes on behalf of the patients and communities served. With recent payer program changes, more questions have been raised as to how practices can continue to support the work of care managers. We need to improve efficiency through standardization, sharing best practices and increased reporting of PDCM encounters.
