It’s A Family Affair: Strengthening Clinical Partnerships with Family Caregivers
Ongoing community living for persons with advanced chronic disease often depends on the support of family. Family care partners frequently coordinate care and assist with other tasks necessary for independent living. However, the demands of caregiving can be physically, emotionally, and psychologically taxing–leading to multiple negative health outcomes for the caregiver and the family unit. Early identification of care partners, including patients serving as caregivers for other relatives as well as family members supporting the patient's care needs at home, can enhance care delivery and improve family health. This presentation will examine family health considerations and provide strategies for strengthening family engagement in care planning.
