Collaborative Care Model: Base Training

THIS REGISTRATION IS FOR BEHAVIORAL HEALTH CARE MANAGERS (BHCMs) AND OTHER PRACTICE STAFF

Collaborative Care Model (CoCM): Base Training for Behavioral Health Care Managers (BHCMs) and other practice staff is designed for Patient-Centered Medical Home (PCMH) designated practices that have completed the CoCM practice assessment and are determined to be ready to implement the model. Other practice staff include but not limited to primary care champions, physician organization representatives, and practice management. The training is intended to help primary care physicians, psychiatric consultants, BHCMs and practice team members gain the foundational knowledge and skills for implementing CoCM. After completing this activity, participants will be able to translate key processes within their practice setting and integrate the COCM model into patient visits.

This training occurs over three days. 

  • Day One: September 12 (8:00 am - 2:15 pm)
  • Day Two: September 14 (8:00 am - 12:30 pm)
  • Day Three: September 19 (8:00 am - 12:00 pm) 

Please note: Training is only for those individuals planning to actively participate in the BCBSM CoCM Program.
