During the October Care Manager Meeting, care managers will learn about practice workflows and team-based approaches that support care management within daily office operations. Tina Lewis, RN Care Manager from Grand Traverse Children’s Clinic, will share examples of office workflows, team collaboration, huddles, staff utilization, and care management processes. Care managers will also follow up on end-of-year topics and opportunities reviewed at the September meeting, discuss processes practices have started or implemented, identify what is working, and share barriers, questions, and additional opportunities. The meeting will include practical discussion of patient and staff education and engagement strategies, including monthly education materials and opportunities to introduce care management through existing practice workflows such as new patient paperwork. Additional education may include a guest speaker, Motivational Interviewing, or another topic based on current care manager needs.
