Specialist Team-Based Care

The Specialist Team-Based Care course is designed to provide care team members with an understanding of the Blue Cross Blue Shield of Michigan Specialist Team-Based Care Program and how teams work to support success in this program. Learn about processes and communication between Primary Care and Specialist offices to improve care coordination and patient outcomes.

Learning Outcome: Learners will be able to describe two processes that are recommended to support their success in the BCBSM Specialist Team-Based Care Program.

The course will take 90 minutes to complete. Successful completion of the Specialist Team-Based Care course requires:

  1. Viewing the entire course (below).
  2. Completing the post-test and evaluation and achieving a passing score of 80% or more.
    • You may retake the post-test if needed. 
    • As part of the evaluation, you will be required to attest to having viewed the course in its entirety.

Upon successful completion of the Specialist Team-Based Care course, you may request Continuing Education contact hours for Nursing, Social Work, or a certificate of completion.
