Beginning June 10, 2020, BCBSM will hold a series of six webinars to introduce interested physician organizations to the Psychiatric Consult Collaborative Care Model.
CoCM is built on a team of three practitioners: 1) a primary care physician, who directs all care; 2) a behavioral health care manager, who assess and screens patients, provides brief patient interventions, regularly meets with a psychiatrist, and liaises between the PCP and psychiatrist; and 3) a psychiatrist, who regularly consults with the BHCM weekly on the patient cases. If your PO is considering using the Collaborative Care Model in the next 12 months, we hope you will join for as many webinars as possible, although attending the webinars isn’t required. Although the programs are designed for PO leaders and staff, POs are welcome to invite practices to attend.
BCBSM has posted the dates and times on the PGIP Calendar.
