Optimizing Medication Reconciliation: Role of Care Team Members (2-Day Training)

This training will focus on medication reconciliation best practices across multiple care team settings, including primary care, specialty, care management focused visits, and transitions of care. A description of various team member roles will also be discussed in order to optimize the care they provide for patients in these settings. This includes care managers such as nurses, social workers, dieticians, and pharmacists, in addition to other care team members such as medical assistants and community health workers. By the end of this training, attendees should be able to:

  • Define medication reconciliation components across different care settings
  • Compare and contrast roles of various care team members in relation to medication reconciliation
  • Discuss how to optimally perform a medication reconciliation
  • Identify red flags and high-risk medications that may impact patient safety
  • Describe the importance of medication reconciliation within transitions of care encounters
  • Review components of a transitions of care phone visit

Successful participants will receive a certificate of completion. Please see registration page for more details.
CEs will not be provided. 

  • Describe medication reconciliation components and the significance of medication reconciliation
  • Discuss the specific role of medical assistants with regard to medication reconciliation
  • Review medication reconciliation best practices
  • Identify red flags and high-risk medications that may impact patient safety
