Michigan Center for Clinical Systems Improvement (Mi-CCSI) has been approved by the Michigan Institute for Care Management and Transformation (MICMT) to provide the MICMT Introduction to Team-Based Care course training. This course is required to bill care management/care coordination codes.
During this live, all-day training course the learner will gain knowledge and skills to work collaboratively in a multidisciplinary setting to assure that care is coordinated, safe, and patient-centered. Open to all members of the practice to gain foundational knowledge in Team-Based Care. Introduction to Team-Based Care will include:
• Why, What, Who and How: Team-Based Care
• Care Management Process
• Outcomes and Triple AIM
• Billing Applications
At the conclusion of this training course, the participant will be able to:
- Review the historical models building to team-based cage, challenges and benefits of a multi-disciplinary care team
- Review challenges, tools (SBAR), and formal/informal team meeting formats
- Identify the multi-disciplinary care team and how each role assist in the care management process
- Review how effective team-based care supports quality outcomes
- Identify how the triple aim supports quality improvement within the care team
- Review the billing codes available to team-based care actions
- Apply team-based care communications, processes and workflows role play a patient visit from start to finish.
- Share ideas and approaches to apply the information within the practice and across the care team to improve patient care.
Intended Audience: This course is intended for care team members who are new to care management or care coordination, or seeking to expand their knowledge and skills as part of a Patient Centered Medical Home.
