Jackson Health Network was the recipient of the Visioning Award for Social Determinants of Health. The goal of this award is to connect people with complex social needs to an established and coordinated community system of care. JHN has accomplished this work in its primary care practices, but operationally, processes in these settings vary greatly from those in the Emergency Department and the acute care hospital.
By embedding a CNS in these locations, people in a higher-risk medical environment will be presented new opportunities for engagement and success as they experience the value of navigation assistance while in a vulnerable state of health.
